AI-Planned, Guide-Transferred Implant Surgery
AI Guided Implant Surgery in Istanbul: Digital Implant Planning Turned into a Precise Surgical Guide
With AI guided implant surgery, your implant positions are planned in a virtual 3D copy of your jaw first, then transferred to your mouth through a custom 3D-printed guide. Every plan and every drilling step is reviewed and carried out by Dr. Abdurrahman Öztürk.
Medically reviewed by Dr. Abdurrahman Öztürk, PhD, oral & maxillofacial surgeryThe model shows how a printed guide sits over the teeth and gums, with a sleeve directing the drill to the implant site planned in the virtual jaw beneath it.
Short answer
What is AI guided implant surgery?
AI guided implant surgery is a method where implants are first positioned virtually on an AI-segmented 3D model of your jaw, built from a CBCT scan and an intraoral scan. A custom 3D-printed surgical guide then steers the drill to that planned angle and depth. The surgeon controls every step and can adjust the plan during surgery.

From Scan to Guide: How AI Guided Implant Surgery Is Prepared
Guided surgery is only as good as the plan behind it. These are the six stages your case passes through before the first drill touches bone, each one checked by Dr. Öztürk.
- 1
Data capture
A CBCT scan records bone in three dimensions and an intraoral scanner records teeth and gums. Together they form the raw material of the plan.
- 2
AI segmentation
Software separates jawbone, teeth, the mandibular nerve canal and the sinus floor into labelled structures, so critical anatomy is visible at a glance rather than hunted for slice by slice.
- 3
Scan registration
The surface scan and the CBCT are aligned on shared landmarks. The doctor verifies the match, because a poor overlay would carry an error into the guide.
- 4
Prosthetic-first design
A virtual tooth is set where your new crown should be. Implants are then positioned underneath it, so the screw serves the tooth instead of the tooth adapting to the screw.
- 5
Virtual implant placement
Length, diameter, angle and depth are chosen implant by implant, keeping safe distances from nerves, roots and the sinus. The doctor approves or edits every suggestion.
- 6
Guide design and printing
A guide with metal sleeves is designed on the approved plan and 3D-printed in biocompatible resin, then test-fitted in your mouth before surgery begins.

AI image analysis
See how AI reads your scan before the doctor decides
Every panoramic X-ray, CBCT scan and smile photo you send is analysed by AI first. It maps bone, nerves, roots and soft tissue, flags areas that need attention and prepares measurements. Dr. Abdurrahman Öztürk then reviews every finding and makes the clinical decision.
What AI Guided Implant Surgery Actually Involves
In conventional implant surgery the dentist studies the X-rays, opens the gum, looks at the bone and decides the final drilling direction in the chair. Skilled surgeons do this well, but the decision is made with the patient already numb and the site already open. AI guided implant surgery moves that decision to the planning stage. Your implant is placed first in software, on a three-dimensional model of your own jaw, and the surgery becomes the execution of a plan that was studied calmly, measured and approved in advance.
The link between the screen and your mouth is the surgical guide: a rigid, custom-printed template that fits only your teeth or gums. It contains metal sleeves, small tubes that direct each drill along the planned axis and stop it at the planned depth. Artificial intelligence speeds up and sharpens the planning, especially the recognition of nerves, sinus and bone boundaries, but it never drills, never decides alone and never replaces the surgeon's judgement during the operation.
Guided does not mean robotic
A guided procedure is still performed by hand. Dr. Öztürk holds the handpiece, controls speed and irrigation, feels bone density and can abandon the guide if the clinical situation differs from the scan.
Where AI fits in
AI is used in image analysis and planning: segmenting structures, flagging thin bone, measuring distances and suggesting implant positions that the doctor then accepts, edits or rejects.
Fully Guided, Pilot Guided or Freehand: Choosing the Level of Guidance
Not every case needs the same degree of guidance, and an honest plan says which one is used and why. In a fully guided protocol, every drill in the sequence, and often the implant itself, passes through the guide sleeve, so angle and depth are controlled from start to finish. In a pilot-guided protocol, only the first narrow drill is guided; the surgeon then widens the site freehand, following the pilot channel. Freehand placement, planned digitally but performed without a template, remains appropriate for some simple sites with generous bone and clear visibility.
The choice depends on bone volume, closeness to the nerve or sinus, the number of implants, whether teeth will be attached immediately, the implant system and how wide you can open your mouth. During your planning consultation Dr. Öztürk explains which level is recommended for each implant position, so you know exactly what kind of surgery you are agreeing to.
- Fully guided: most control, useful near critical anatomy and for immediate teeth
- Pilot guided: controls the starting direction, keeps more tactile freedom
- Digitally planned freehand: plan studied in 3D, surgery done without a template
- Mixed approach: different levels can be used for different sites in one mouth
Types of Surgical Guides and How Each Is Supported
A guide is only accurate if it sits in exactly the same position during surgery as it did in the software. That is why guides are classified by what holds them in place. The support type is chosen during digital planning according to how many of your own teeth remain and how firm the gums and bone are. A guide that rocks even slightly can turn a precise plan into an imprecise result, so fit is checked before any incision or drilling begins.
When several implants are planned in a jaw with few or no teeth, small fixation pins may be used to lock the guide to the bone for the duration of surgery. These are removed at the end and the tiny holes they leave heal on their own. In full-arch cases a sequence of stackable guides may be used, for example one for any bone reshaping, one for the implants and one for the temporary teeth.
Tooth-supported guides
Used when healthy teeth remain next to the gap. Resting on hard enamel, they are generally the most stable and the most accurate type.
Mucosa-supported guides
Used in jaws without teeth. They rest on the gums and are usually anchored with fixation pins, because soft tissue can compress and allow movement.
Bone-supported guides
Placed directly on bone after the gum is opened. Today they are used less often, mainly when bone needs to be reshaped or visualised during surgery.
How Accurate Is AI Guided Implant Surgery? Realistic Expectations
Guided surgery is widely described in dental research as more predictable than freehand placement, particularly for angle and position, but it is not perfectly error-free. Small deviations can arise at every link of the chain: the resolution of the CBCT, the alignment of the two scans, the printing of the guide, the tolerance between drill and sleeve, the seating of the guide and the way bone of different density behaves under the drill. Each of these errors is small, yet they can add up.
This is why a responsible plan always keeps a safety margin around the nerve, the sinus floor and neighbouring roots instead of planning to the very edge. A commonly taught principle is to keep implants about two millimetres away from the inferior alveolar nerve; the exact distance in your case is decided by the doctor. AI helps by marking these structures clearly and measuring distances automatically, but the margins are clinical decisions. After placement, an X-ray or scan can confirm the final implant position relative to the plan.
- Planning accuracy depends on scan quality: patient movement during CBCT blurs the image
- Metal crowns and fillings can create scan artefacts that must be corrected
- Guide fit is verified in the mouth before surgery starts
- The surgeon can switch to freehand if the guide cannot be seated safely
Flapless Surgery and Same-Day Teeth with a Digital Plan
Because the bone has already been studied in three dimensions, guided implant surgery can sometimes be performed flapless: instead of cutting and lifting the gum, a small circular opening is made exactly where the implant will go. Many patients experience less swelling and a quicker return to normal routine with this approach, and stitches may not be needed. Flapless placement is not always possible, however. When the band of firm gum is thin, when bone needs to be added or reshaped, or when the surgeon needs direct vision, a conventional small flap is the safer option.
A digital plan also allows temporary teeth to be designed before surgery. If the implants achieve sufficient primary stability, measured during placement, a provisional crown or bridge prepared on the same plan can sometimes be fitted the same day. Whether immediate teeth are possible is only confirmed during the operation, never promised in advance, because stability depends on the bone found in the chair.
Who Benefits Most from Guided Implant Placement
Guided surgery adds the most value where the margin for error is small or where several implants must work together. Typical examples are implants close to the mandibular nerve in the lower back jaw, implants under the sinus in the upper back jaw, narrow bone ridges where angle matters, front teeth where the aesthetic position of the crown is critical, and full-arch treatments where implants must be parallel enough to carry a single bridge. It is also valuable for patients with limited time in Istanbul, because a precise plan and a pre-designed temporary make the surgical day more efficient.
Guided surgery is less suitable in some situations. Patients who cannot open their mouth wide may not have enough space for the guide plus the drill, especially at the back molars. Sites needing extensive bone grafting may be better treated in stages. A single implant in abundant bone may not gain much from a full guide. Dr. Öztürk will tell you if a guide is unnecessary in your case rather than adding one by default.
- Limited mouth opening can make guided drilling at the back difficult
- Uncontrolled diabetes, heavy smoking and active gum disease affect implant healing whatever the technique
- Recent radiotherapy to the jaw and certain bone medications require special assessment
Your Guided Implant Surgery in Istanbul as an International Patient
Most of the preparation for guided implant surgery can begin before you travel. Send your photos and any existing X-rays or CBCT scans to the clinic on WhatsApp; they are analyzed with AI-assisted tools and Dr. Öztürk gives a preliminary view on whether guided surgery is likely to suit you and how many visits you may need. Recent CBCT scans taken elsewhere can sometimes be used, but a new scan is taken in Istanbul if the existing one is outdated, low resolution or does not cover the required area.
Once you arrive, the scans, planning meeting and guide production usually take a few days before surgery. Guide printing and verification are part of the schedule, not an afterthought. After surgery, implants need several months to integrate with the bone before final crowns are fitted, so most international patients plan two trips. Your doctor will give you a written timeline for your specific case.

Typical Timeline for AI Guided Implant Surgery
Before travel
Online scan review
Send photos and X-rays or CBCT on WhatsApp for an AI-assisted preliminary assessment by Dr. Öztürk.
Day 1
Examination and scanning
Clinical exam, new CBCT if required and an intraoral 3D scan; AI segmentation of your jaw begins.
Day 1–3
Virtual planning and guide
You review the implant plan in 3D with the doctor. After approval the guide is designed, printed and test-fitted.
Surgery day
Guided implant placement
Implants are placed under local anaesthesia through the guide; a temporary is fitted only if stability allows.
About 1 week
Early check
Healing is checked and a control X-ray confirms implant positions before you fly home.
After healing
Final teeth
Once the implants have integrated, usually after several months, final crowns or bridges are made, often from digital scans.
Why patients choose it
Plan before you commit
You see your implant positions in a 3D model of your own jaw and can ask questions before any surgery is booked.
Respect for critical anatomy
AI-segmented nerves and sinus floors with measured distances help the surgeon keep safe margins around delicate structures.
Tooth-driven positioning
Implants are placed where the future crown needs them, which supports better aesthetics, cleaning access and bite forces.
Potentially gentler surgery
In suitable cases a flapless approach means a small opening instead of a full gum incision, often with less swelling.
Efficient surgical day
Because decisions are made in advance, time in the chair can be shorter and more predictable for travelling patients.
Ready-made temporaries
Provisional teeth can be prepared from the same plan, so immediate teeth are possible when implant stability allows.

Questions and answers
What patients ask about AI Guided Implant Surgery
Is AI guided implant surgery more accurate than normal implant surgery?
In most cases it is more predictable, especially for angle and depth, but it is not perfect. Research generally shows guided placement deviates less from the plan than freehand placement. Small errors can still come from scans, printing and guide fit, so the doctor always keeps safety margins around nerves and the sinus.
Does the AI or a robot drill my implants?
No. A surgeon drills by hand at AIDENT. AI analyzes your CBCT, marks anatomy and suggests implant positions; the printed guide then steers the drill. Dr. Öztürk controls the handpiece, the speed and every decision, and he can change the plan during surgery.
Can implants be placed without cutting the gum?
Sometimes, yes. Flapless guided surgery uses a small round opening at the implant site instead of an incision. It needs enough firm gum and bone and is not suitable when grafting or direct vision is required. The doctor decides after studying your 3D scan.
Is guided implant surgery painful?
The surgery itself is not painful because it is done under local anaesthesia. Afterwards most people feel mild to moderate soreness for a few days, controlled with ordinary painkillers. Flapless cases often have less swelling, but every patient heals differently.
How long does it take to make a surgical guide?
Usually a few days from scanning to a test-fitted guide. The plan must be designed and approved, then the guide is 3D-printed, cleaned, prepared and checked in your mouth. International patients should allow for this in their Istanbul schedule before the surgery day.
How much time do I need to stay in Istanbul for guided implant surgery?
Plan roughly one week for the first trip, covering scans, planning, guide production, surgery and an early check. Final crowns are usually fitted on a second trip after the implants have integrated with the bone, typically several months later. Your exact schedule depends on your case.
Can I send my own CBCT scan before I come?
Yes, please do. Send your CBCT files or images on WhatsApp for a preliminary AI-assisted review. If the scan is recent, sharp and covers the required area, it may help planning; otherwise a new CBCT is taken in Istanbul to make a reliable guide.
What is the difference between a fully guided and a pilot-guided implant?
A fully guided protocol guides every drill, and often the implant, through the template. A pilot guide controls only the first narrow drill, after which the site is widened freehand. Fully guided offers more control; pilot guided keeps more flexibility. The choice depends on your anatomy.
Who is the best dentist for AI guided implant surgery in Istanbul?
Look for a surgeon with formal surgical training who plans and performs the guided surgery personally. At AIDENT Clinic, Dr. Abdurrahman Öztürk, who holds a PhD in oral and maxillofacial surgery and has 15+ years of experience, reviews every AI plan and operates himself.
Can I get teeth on the same day with guided surgery?
Sometimes. Because the temporary can be designed from the digital plan in advance, same-day teeth are possible when the implants are stable enough at placement. Stability is measured during surgery, so immediate teeth are never guaranteed beforehand.
Is guided surgery suitable if I cannot open my mouth wide?
It can be difficult. The guide and the long drills need vertical space, which is often limited at the back molars. The doctor measures your mouth opening at the examination and may choose a pilot guide, shorter drill systems or freehand placement for those sites.
Does guided surgery work if I need a bone graft?
It depends on the graft. Small defects can often be handled during guided surgery. Larger grafts or sinus lifts may require opening the gum or a staged approach, where bone is built first and implants are placed with a guide months later.
Does AI really make implant planning safer?
AI makes important anatomy easier to see and measure, which supports safer planning. It quickly highlights the nerve canal, sinus and thin bone. However, safety still depends on the surgeon checking the AI output, keeping margins and handling surprises in the chair.
What happens if the guide does not fit on surgery day?
The surgeon does not drill through a guide that does not seat firmly. Fit is tested before anaesthesia work begins. If it cannot be corrected, the doctor may continue with digitally planned freehand placement or postpone and remake the guide, depending on what is safest.
